I, A.B. Official Liquidator of the above-named company, solemnly affirm and say as follows:.

1.      The contributories of the said company, whose names are set forth in the schedule to the summons herein marked .A., have not paid the sums set opposite their respective names in the said schedule, which sums are the amounts now due from them respectively under the call of Rs_______ per share duly made under the Companies Act, 1956, pursant to the order of this Court, dated_______ granting leave to make a call.

2.     A copy of the said, order, dated ______ and a notice in the prescribed form specifying the amount due from each of the said contributories were duly served by registered post (or as the case may be) on such contributories respectively.

3.     The respective amounts set opposite the names of such contributories respectively in the sixth column of the said schedule are the true amounts now due and owing by them respectively in respect of the said call.
