Request by person designated to nominate arbitrator for nominee to act
Dear Sir,

The President of the [Institute or Association or as the case may be] has been asked to appoint an arbitrator in the above matter and has asked me to ascertain whether you are willing to act. So that you may know the nature of the case I enclose a copy of the application and of the relevant papers for your information.
I shall be obliged if you will let me know as soon as possible if you are willing to accept the appointment in which case I will send you the original form of application for signature.
You will of course appreciate that if you are in any way related to or connected with the parties or otherwise concerned with the subject-matter it would be unwise for you to accept.
If for any reason you are unable to act will you please return the copy of the application and the papers to me.
